Parent at Pediatric Dentist

Thank you for choosing Pediatric Dental Center of Mansfield and Bridgewater and welcome to our wonderful family of patients.

A healthy smile helps maintain the health of the entire body. Our friendly and knowledgeable team will provide you with the highest level of care and attention at each and every visit. Please take a moment to read our office policy below to prepare for your child’s dental visit.

As set forth by the management, all fees for private accounts (i.e., no insurance) must be paid in full at the time of service. Payment can be made by credit card/debit card, check, or cash.Other payment arrangements must be authorized in advance. Any account balance of 30 days or more will be subject to a service charge on the unpaid balance. Those individuals with insurance must pay co-payments and/of deductibles at time of service. Any insurance payment not received in 45 days from the date of service will be the insured responsibility. We accept no responsibility in collecting overdue insurance claims or negotiating settlement on disputed claims. 

 

PLEASE NOTE: IT IS THE RESPONSIBILITY OF THE INSURED TO KNOW THEIR BENEFITS

 

Please inform us of any insurance changes that may occur during your child’s treatment at this office.

 

Our office is contracted with the following dental insurance companies; Delta Dental, Blue Cross-Blue Shield, Cigna, United Concordia and Altus. We are able to submit to other insurances but will be considered "out-of-network," so your fee may be slightly higher.

Should you need to set up a payment plan, applications for Care Credit are available at the office.

Cancellations: We require 24 hours notice for cancellations. If cancellations occur frequently, we reserve the right to charge your account. We only ask that you respect our time as much as we respect yours.
